Intestinal permeability ("leaky gut") is a real, measurable, clinically significant phenomenon — documented in peer-reviewed research and associated with multiple chronic diseases. But most of what the supplement industry calls "leaky gut" is marketing fiction attached to a real mechanism. Here's what's actually true.
Your intestinal wall is a single layer of epithelial cells — a barrier roughly 1 cell thick separating your gut contents from your bloodstream. These cells are held together by protein structures called tight junctions. When tight junctions are intact, they allow selective passage of nutrients while blocking bacteria, bacterial fragments (lipopolysaccharides/LPS), and undigested food proteins from entering the bloodstream.
When tight junctions degrade — due to inflammation, dysbiosis, certain dietary factors, or disease — the barrier becomes "leaky." Bacterial fragments and larger molecules pass through into the bloodstream, triggering an immune response. This systemic immune activation is what produces the downstream effects attributed to intestinal permeability.
This is not a fringe concept. It's published in Nature, The Lancet, and Cell. It has robust mechanistic evidence and is associated with Crohn's disease, celiac disease, type 1 diabetes, and — with growing evidence — metabolic syndrome, neuroinflammation, and liver disease.
Overgrowth of pro-inflammatory bacteria (particularly LPS-producing gram-negative bacteria) directly degrades tight junction proteins. The LPS released from these bacteria also directly stimulates intestinal inflammation, creating a self-amplifying cycle.
One of the strongest evidence-based causes. Alcohol directly dissolves tight junction proteins and promotes bacterial overgrowth. Alcoholic liver disease is substantially mediated by intestinal permeability — LPS entering the portal circulation drives hepatic inflammation.
Non-steroidal anti-inflammatory drugs increase intestinal permeability within hours of ingestion by inhibiting prostaglandin synthesis, which is required for tight junction maintenance. Chronic NSAID use is strongly associated with increased gut permeability.
Stress hormones (particularly corticotropin-releasing hormone/CRH) directly loosen tight junctions via mast cell activation. This is the mechanism behind "stress stomach" — anxiety literally makes the gut more permeable, which worsens gut symptoms, which increases anxiety. A documented bidirectional cycle.
Emulsifiers (particularly carboxymethylcellulose and polysorbate-80, used widely in processed foods) disrupt the mucus layer overlying the intestinal epithelium and alter microbiome composition toward pro-inflammatory species. A 2021 Cell study found significant intestinal permeability increases in humans consuming ultra-processed food diets.
"Gluten causes leaky gut in everyone"
Gluten triggers measurable intestinal permeability in people with celiac disease and non-celiac gluten sensitivity. In healthy adults without these conditions, the evidence for significant gluten-induced permeability is weak. The mechanism (zonulin release) is real but the effect size in non-susceptible people is small and transient.

Associations between intestinal permeability and these conditions exist in research, but association is not causation. Whether permeability is a cause, a consequence, or a parallel effect of these conditions is largely unresolved. The gut-brain axis connection is real; the direct causal chain is not yet established for most psychiatric conditions.
"You need an expensive supplement protocol to fix leaky gut"
The most evidence-backed interventions for intestinal barrier integrity are dietary: fiber diversity, fermented foods, omega-3 fatty acids, zinc, and reducing ultra-processed food and alcohol. Specific supplements (L-glutamine, collagen) have some evidence but are secondary to the dietary foundation. No supplement overcomes a diet that chronically disrupts the barrier.
Clinical testing exists: the lactulose/mannitol ratio test measures how much of each molecule crosses the intestinal barrier after oral dosing. Elevated lactulose:mannitol ratio indicates increased permeability. Zonulin levels (a protein that regulates tight junctions) can be measured in serum and stool. These tests are available through functional medicine physicians and some gastroenterologists.
Practically: if you have IBD, celiac disease, a history of heavy NSAID use, chronic high stress, or consume significant amounts of alcohol and ultra-processed food, your barrier function is likely compromised to some degree. The interventions above are appropriate to implement regardless of testing.
