Phase 1Elimination (2–6 Weeks)
1Remove all high-FODMAP foods completely. This means: garlic, onion (in any form, including powder), wheat, rye, barley, most legumes (lentils, chickpeas, black beans), apples, pears, mangoes, peaches, watermelon, stone fruits, milk, ice cream, soft cheeses, mushrooms, cauliflower, and all artificial polyol sweeteners.
2If running a full elimination (beyond low-FODMAP), also remove: all gluten grains, all conventional dairy, eggs, soy products, corn, nightshades (tomatoes, peppers, eggplant, white potatoes), alcohol, and processed foods with long ingredient lists.
3Eat plenty — this is not a calorie-restriction diet. Focus on: rice, quinoa, potatoes (sweet and Yukon Gold are low-FODMAP), all meats and fish, eggs (if not doing full elimination), low-FODMAP vegetables (carrots, zucchini, bell peppers, cucumber, leafy greens, bok choy), berries, citrus fruits, bananas (unripe), lactose-free dairy or coconut milk, and small amounts of most nuts and seeds.
4Check every label. FODMAPs hide in condiments, broths, sauces, and packaged foods. "Onion powder," "garlic extract," "inulin," "chicory root," and "high-fructose corn syrup" are all red flags.
5Maintain your food and symptom journal daily. Note energy levels, bowel frequency and consistency (use the Bristol Stool Scale), bloating severity (rate 0–10), and any extra-intestinal symptoms like skin breakouts, joint pain, or headaches.
6Expect a 1–2 week adjustment period. Some people feel worse initially before the microbiome recalibrates. This is normal. If symptoms dramatically worsen after week two, consult your healthcare provider.
7Aim for symptom stability for at least one week before beginning reintroduction. If symptoms haven't improved after 6 weeks, stop and reassess with a clinician — your issue may not be FODMAP-related.
Phase 2Reintroduction (6–8 Weeks)
1Reintroduce one FODMAP group at a time — never two simultaneously. Standard Monash order: fructans (test with wheat), lactose (test with milk), fructose (test with honey), polyols-sorbitol (test with avocado), polyols-mannitol (test with mushrooms), GOS (test with canned lentils), fructans-veg (test with onion).
2Each challenge lasts 3 days. Day 1: small serving of the test food. Day 2: medium serving. Day 3: large serving. Then return to elimination baseline for 2–3 days to clear symptoms before testing the next group.
3Score your symptoms on each challenge day. A score increase of 3+ points (on a 0–10 scale) that correlates with the test food on 2 of 3 days indicates a sensitivity to that FODMAP group.
4If you react to a food group, do not abandon it permanently. Many sensitivities are dose-dependent. After completing all challenges, you can re-test reactive foods at lower amounts to find your personal threshold.

For each day, log: the time and contents of every meal and snack (including portion sizes, cooking oils, condiments, and beverages), any supplements or medications taken, bowel movements (frequency and Bristol Stool Scale score 1–7), bloating severity on a 0–10 scale, pain location and severity, energy levels, stress levels, sleep quality, and any other symptoms (headache, skin, joint pain, brain fog).
Pro tip: Photograph every meal. It takes seconds and captures information your written notes might miss. You can review patterns visually when analyzing your reintroduction data.
During reintroduction, look for patterns across the 3-day challenge window. If you ate mushrooms on Monday and felt bloated on Tuesday afternoon, that's a mannitol reaction. The lag can be confusing — this is why you return to baseline elimination foods between each challenge. Get tested for celiac disease before eliminating gluten — a celiac antibody panel requires active gluten consumption to be accurate. If you go gluten-free first and then test, you may receive a false negative and never get the correct diagnosis.
